How to Apply
Read this information carefully before opening the application form.

Select referees who can comment on your Research/Academic ability and potential to succeed in a graduate program.
The University will contact your referees using the email information you have provided in your application. It is recommended that you confirm your referee’s availability and email address prior to submitting their contact information. Alert them that an email, with the appropriate link to the reference submission pages, will be arriving from: SGS Graduate Admissions Application.
It is critical that you contact your referees in advance to explain this process so that they can respond appropriately. Also, it may be necessary to follow up with your referees, to ensure that the reference is submitted, electronically, by the deadline.
Official TOEFL/TWE (if applicable) – submitted directly from testing centre.
Codes for TOEFL/TWE: 0982 (University of Toronto)
Order Official Transcripts from ALL undergraduate and graduate programs to be sent directly by the issuing institution to the Rehabilitation Sciences Institute.
A Letter from potenial supervisor indicating financial and supervisory support.

Official Transcripts – mail to Rehabilitation Sciences Institute.
For transcripts from Canadian universities only : In lieu of a scanned copy of a paper transcript, applicants may upload a .PDF file of their academic history from their university's student web service. Where possible, the file should include the university's grading legend and your name. If the admissions committee determines that the file is incomplete, you may be asked to submit a copy of the transcript instead.
Internal applicants do not need to send original University of Toronto transcripts as ROSI print-outs are acceptable.
